This code is applicable if you are a part of the Health Care Connect Program and accepting a patient, through the HCC program, deemed as being a complex and/or vulnerable patient. 8. Weeneebayko Health Ahtuskaywin (WHA)
9. Comprehensive Care Model (CCM); and
10. Family Health Group (FHG).
